VCH Local Assessment and Treatment Matching Team / Intake information:
* A person can self-refer by calling our Assessment and Treatment Matching Team at the above numbers below.
Pemberton: (604) 698-5861, available Monday to Friday from 8:30 a.m. to 4:30 p.m.
Squamish: (604) 815-3008, available Monday to Friday from 8:30 a.m. to 4:30 p.m.
Whistler: (604) 698-6455, available Monday to Friday from 9 a.m. to 5 p.m.
We also accept faxed referrals on our Referral Form from Family doctors, emergency department and other community partners.
Please note – referrals for psychiatry services require a written referral from a physician.
